East Coast Parti Yorkies Purchase Date __________Purchase Price_______Deposit________Breeding Rights________ On this date East Coast Parti Yorkies is selling a _______Gender, ___________________Breed _______________________________ (Sire) and ________________________________ (Dam) born on_______________________ and with AKC# ___________________________________ which are marked with (limited or unlimited) breeding rights. For the Health Guarantee to be in effect you will need to take your puppy to a veterinarian within the 3 days of Purchase for a well puppy visit. All Sundays and holidays are not counted because veterinarians offices are not Usually open for business on these days. We know that 3 days is not allot of time, but this way you or a family member does not develop a strong bonding to your new baby. If you do not have a well puppy visit within the 3 days this Health Guarantee is voided. If within the 3 days your puppy is diagnosed with a life threatening health problem such as heart, liver, kidney the puppy can be returned for a full refund of the purchase price of the puppy only. If the puppy dies within the 3 days of purchase the buyer must have an autopsy performed to determine the cause of death. The purpose of the autopsy is to verify that the puppy did not die due to injury, accident or an illness that was caused by the buyer. A copy of the report must be forwarded to the seller before determining if a refund will be given. In order to return a puppy you must have the following: Provide a signed statement from your veterinarian showing the date, within the 3 day period when the puppy went for his/her well puppy visit. The reason why the vet believes the puppy has a life threatening health problem that existed prior to you receiving the puppy. If the buyer chooses to keep the puppy, regardless of any health issue the seller will not issue a refund for the cost of the puppy. If another puppy is available the value greater than the original puppy’s purchase price the buyer is responsible for the difference in the cost. If the buyer chooses a replacement puppy that has a value less than the original purchase price the buyer will be issued a refund for the difference. These are not covered by our Health Guarantee: *Worms & Parasites, including Coccidia, Giardia (common in puppies). All puppies are dewormed at 2,4,6,8 weeks old. But additional treatment may be necessary. We recommend bringing a stool sample with puppy to his/her well puppy visit. *We do not guarantee a dog that develops Hypoglycemia, retained baby teeth, hernias. *We do not guarantee the puppy to have any breeding potential. *Non-life threatening Health problems.
